LETTER OF AGENCY AND AGREEMENT
To the Incumbent Local Exchange Carrier (ILEC),
The undersigned appoints Arizona Dialtone, Inc. (AZDT) as agent to order changes
to and maintenance on specific telecommunications services you, the ILEC, provided
to the undersigned, including, without limitation, removing, adding to or rearranging
such telecommunications service.
You, the ILEC, are hereby released from any and all liability for making pertinent
information available to AZDT and for following AZDT's instructions with reference
to any changes or maintenance on the undersigned's telecommunications service.
You, the ILEC, may deal directly with AZDT on all matters pertaining to said telecommunications
services and should follow AZDT's instructions with reference thereto. This
authorization will remain in effect until otherwise notified in writing by both parties.
In closing, I have read, understand, and agree to all terms and conditions.
